HESI Exit exam guide: format, scores and a study plan

What the HESI Exit exam measures, how schools use the score, and a four-week plan using original HESI-style practice questions.

Last reviewed October 21, 2026 · Naxlex RN Hub study team

The HESI Exit exam is a comprehensive test that many nursing programs give near graduation to check NCLEX readiness. Schools decide how the score is used, including progression or remediation rules. Preparation looks a lot like NCLEX prep: clinical judgment across every body system.

The Exit exam covers the whole curriculum: medical-surgical, pharmacology, maternity, pediatrics, mental health, fundamentals and leadership. Questions are written at the application and analysis level, so knowing facts isn't enough. You have to decide what to do with them.

Elsevier reports results on a scaled scoring system. Many programs set a benchmark, often around 850 or 900. Your own program's policy decides what happens if you score below it, so read it early.

A simple four-week plan works well. In week 1, take a baseline exam and list your three weakest areas. In weeks 2 and 3, do daily focused quizzes on those areas and review the rationales. In week 4, take a full timed simulation and do light review. Sleep matters more than one more set the night before.
