Free NCLEX practice questions with rationales (RN and PN)

Try original NCLEX-style practice questions on prioritization, pharmacology and safety, each with a full rationale explaining every option.

Last reviewed October 21, 2026 · Naxlex RN Hub study team

Naxlex RN Hub offers free, original NCLEX-RN and NCLEX-PN practice questions. Each one comes with a rationale that explains the correct answer and why each distractor is less safe. Create a free account to keep practicing by topic.

Good practice questions teach a way of thinking, not a list of facts. Every NCLEX item asks one hidden question: what is the safest next action for this client right now? The samples below show how to slow down, find the cue that matters and rule out answers that are true but not a priority.

Read the stem twice. The first time, find the client and the problem. The second time, find the instruction words: first, priority, further teaching needed, or report to the provider. Those words change the answer even when the clinical facts stay the same.

After each question, explain out loud why the wrong options are wrong. If you can't, you guessed. Mark the topic and come back to it in a focused quiz.

Original practice questions

1. A client with heart failure takes furosemide and digoxin. Which laboratory value should the nurse report before giving the morning doses?

  1. Sodium 138 mEq/L
  2. Potassium 3.1 mEq/L
  3. Glucose 104 mg/dL
  4. Calcium 9.2 mg/dL
Show answer and rationale

Answer: B. Low potassium (normal about 3.5–5.0 mEq/L) raises the risk of digoxin toxicity and dysrhythmias, and furosemide lowers potassium further. The other values are within the usual range.

2. The nurse is caring for four clients. Which task is appropriate to assign to an assistive personnel (AP)?

  1. Teach a newly diagnosed client to use an insulin pen
  2. Assess a client reporting new chest pressure
  3. Measure and record intake and output for a stable client
  4. Evaluate a client's response to a PRN analgesic
Show answer and rationale

Answer: C. Measuring intake and output for a stable client is a routine, predictable task within AP scope. Teaching, assessment and evaluation remain with the licensed nurse.

3. A client 2 hours after a thyroidectomy reports tingling around the mouth. What is the nurse's priority?

  1. Offer an ice pack for the incision
  2. Assess for Chvostek and Trousseau signs and notify the provider
  3. Encourage the client to rest their voice
  4. Document the finding as an expected effect of anesthesia
Show answer and rationale

Answer: B. Perioral tingling suggests hypocalcemia from parathyroid injury, which can progress to tetany and laryngospasm. Assess for it and report it promptly; IV calcium gluconate may be needed.

FAQ

Are these real NCLEX questions?

No. They are original items written in the NCLEX style. Live or recalled exam questions are not legitimate study material.

How many practice questions should I do a day?

Many students do 75–150 questions a day while reviewing every rationale. Thorough review matters more than the number of questions.

Are there free NCLEX-PN questions too?

Yes. Choose the PN track for practice built on the PN test plan, including Coordinated Care.

Where can I get more free questions?

Create a free Naxlex RN Hub account. Free learners get sample quizzes in every section, and CAT practice stays free.
